I hereby acknowledge and agree that the participation in the activities offered at Cornerstone Church of Nashville, Inc., located within the Cornerstone Church of Nashville, Inc. complex, have inherent risks. In consideration of the services provided by Cornerstone Church of Nashville Inc., their agents, officers, directors, participants, consultants, employees and all other persons or entities acting in any capacity on their behalf (hereinafter collectively referred to as Church), I (the participant, if 18 years of age or older, or parent or legal guardian of the listed minor participant) now agree and certify as follows:

1. I acknowledge and fully understand that the Participant, will be engaging in the physical activity that may involve risk of serious injury, which might result from participation, inactions or negligence, but also from the actions, inactions or negligence of others or the conditions of the premises or of any equipment used. Further, that there may be other risks not known or not reasonably foreseeable at this time. The risks may include, but are not limited to nature of these activities, latent or apparent defects of conditions in equipment or property supplied by Church or another entity, acts of other Participants in these activities, employees, volunteers or agents of the Church, Participants’ physical condition , acts or omissions, conditions of the Church facility and surrounding grounds or terrain and accidents connected with their use, first aid emergency treatment or other services.

2. I expressly agree and promise to accept and assume all the risks existing in these activities. Participation in these activities are purely voluntary and I elect to authorize the Participant to participate in spite of risks associated with the physical activity, which could include injury and/or death. I assume all risks and accept all personal responsibility for all damages following any such injury.

3. On behalf of myself, my spouse, my children, my parents, my heirs, assigns and/or personal representatives, I hereby voluntarily release, waive, forever discharge and agree to identify and hold harmless the Church and each of its officers, directors, agents, sponsors, employees, volunteers, etc. and if applicable, operator or lessors of premises used to conduct the event/ activity, from any and all liability for any and all claims, demands, causes of action, losses or costs, including attorney's fees, which are in any way, connected with any participation in these activities or my use of the Church’s equipment, premises or facilities.

4. I hereby certify that I have adequate insurance to cover any injury or damage that may cause or suffer during participation in these activities or alternatively, I agree to bear the cost of such injury or damage myself. I further certify, that the participant has no medical or physical conditions, which could interfere with safety in these activities or else I am willing to assume and bear the costs of all the risks that may be created, directly or indirectly, by any such condition.

5. I acknowledge and certify, that I am at least 18 years old or the legal parent/ guardian of the participant under 18, I hereby provide the Church permission to administer First Aid and I authorize its agents or employees to contact 911 or other emergency personnel as needed.

6. Photos may be taken during these activities for promotional and publicity purposes. I give the Church permission to take and use the photos for promotion/ publicity purposes.

7. By agreeing to this document, I acknowledge, that if anyone is hurt or property is damaged during participation in these activities, I and each of the other parties listed in Item # 3 above waive any and all rights to bring, initiate and/or maintain a lawsuit against the Church, for which I have released them herein.
